Unveiling the Dynamics of Cryptosporidium in Urban Surface Water: A Quantitative Microbial Risk Assessment and Insights into Climatic and Seasonal Influences

In response to global urbanization and economic development, urban surface water pollution has become a universal challenge and particularly affects densely populated megacities, and Dhaka is no exception. The discharge of 98% of untreated domestic sewage and massive volumes of industrial wastewater from over 7000 industries escalate surface water crises. This study investigates microbial and fecal contamination with particular emphasis on Cryptosporidium in surface water, known for causing waterborne diseases, such as cryptosporidiosis. Findings reveal high Cryptosporidium oocyst concentrations and fecal contamination in various water bodies in Dhaka City. Among the investigated water bodies, the Buriganga River exhibits the highest Cryptosporidium oocyst concentration (46%), while the Balu River, Turag River, Shitalakkhya River, Dhanmondi Lake, Gulshan Lake, Banani Lake, Ramna Lake, and Crescent Lake also present high levels of oocyst concentrations ranging from 21–40%. This study also calculated infection risks and found that the infection risk of swimming is highest during the wet season and is (3.9 ± 2.2 (95% CI: 3.0–5.0)) × 10−2 per swimming event, whereas it is approximately (2.4 ± 1.9 (95% CI: 1.6–3.3)) × 10−2 during the dry season. Annual diving risks are approximately (1.2 ± 0.6 (95% CI: 0.9–1.4)) × 10−2, indicating considerably high risks. Most of the sampling sites generally show significantly higher risks than other study areas like the Mymensingh and Kushtia Districts. In light of these results, we strongly recommend immediate measures to address water quality issues and mitigate the risks associated with Cryptosporidium contamination in Dhaka’s surface water.

Applications of Quantitative Microbial Risk Assessment to Respiratory Pathogens and Implications for Uptake in Policy: A State-of-the-Science Review.

Respiratory tract infections are major contributors to the global disease burden. Quantitative microbial risk assessment (QMRA) holds potential as a rapidly deployable framework to understand respiratory pathogen transmission and inform policy on infection control. The goal of this paper was to evaluate, motivate, and inform further development of the use of QMRA as a rapid tool to understand the transmission of respiratory pathogens and improve the evidence base for infection control policies. We conducted a literature review to identify peer-reviewed studies of complete QMRA frameworks on aerosol inhalation or contact transmission of respiratory pathogens. From each of the identified studies, we extracted and summarized information on the applied exposure model approaches, dose-response models, and parameter values, including risk characterization. Finally, we reviewed linkages between model outcomes and policy. We identified 93 studies conducted in 16 different countries with complete QMRA frameworks for diverse respiratory pathogens, including SARS-CoV-2, Legionella spp., Staphylococcus aureus, influenza, and Bacillus anthracis. Six distinct exposure models were identified across diverse and complex transmission pathways. In 57 studies, exposure model frameworks were informed by their ability to model the efficacy of potential interventions. Among interventions, masking, ventilation, social distancing, and other environmental source controls were commonly assessed. Pathogen concentration, aerosol concentration, and partitioning coefficient were influential exposure parameters as identified by sensitivity analysis. Most (84%, ) studies presented policy-relevant content including a) determining disease burden to call for policy intervention, b) determining risk-based threshold values for regulations, c) informing intervention and control strategies, and d) making recommendations and suggestions for QMRA application in policy. We identified needs to further the development of QMRA frameworks for respiratory pathogens that prioritize appropriate aerosol exposure modeling approaches, consider trade-offs between model validity and complexity, and incorporate research that strengthens confidence in QMRA results. https://doi.org/10.1289/EHP12695.

Disentangling the effects of electroporation and heat during pulsed electric field (PEF) processing of oat-based milk alternative: A case study on Lactiplantibacillus plantarum inactivation

The study aimed to quantify the resistance of two L. plantarum strains and estimate the contribution of temperature during the pulsed electric field (PEF) processing of oat beverage. PEF and thermal inactivation kinetic experiments of L. plantarum strains were performed separately. Log10 reductions ≥5.7 of L. plantarum was achieved when 10–24 kV/cm, 110–115 kJ/L, 80–522 μs, and 35 °C inlet temperature were applied, while strain variability was evident in the performed assessments. Accurate monitoring of the temperature profiles for PEF allowed the estimation of the thermal microbial inactivation which was separated from electroporation effect. The electroporation impact was found to increase with increasing electric field strengths. Temperature measurements in the treatment zone confirmed the presence of hot spots. At higher specific energy inputs, there was significant contribution of temperature to the total observed inactivation. This contribution became smaller when the flow rate was increased, as shorter residence time reduced the exposure to high temperature peaks. Industrial relevanceThe findings demonstrated the potential of PEF as a promising technology for the microbial decontamination of oat-based milk alternative. Knowledge of strain variability in response to emerging food processing technologies is critical during process optimisation and quantitative microbial risk assessment. New developments in PEF equipment and process should ideally aim to maximize inactivation while minimizing the impact of high temperature on sensitive food constituents.

Quantitative microbial risk assessment of gastrointestinal illness due to recreational exposure to E. coli and enterococci on the southern coasts of the Caspian Sea

BackgroundGastrointestinal illness refers to a broad range of diseases that affect the digestive system, including infections caused by bacteria, viruses, and parasites. Quantitative Microbial Risk Assessment (QMRA) is a powerful tool used to evaluate the risks associated with microbial pathogens in various environments. The main objective of this study was to conduct a quantitative assessment of gastrointestinal illnesses that occur as a result of exposure to E. coli and enterococci during recreational activities on the southern coasts of the Caspian Sea. MethodsSamples were collected from the recreational beaches along the border line of the Caspian Sea. The samples were analyzed for the presence and enumeration of E. coli and enterococci using the microplate method and membrane filtration techniques. Then, the annual and daily infection risks were computed using the Monte Carlo simulation approach. ResultsThe results revealed that the risk of daily and annual infections on the coasts of Babolsar was higher than that on the coasts of Sari. Also, in the recreational waters of these beaches, the risk of infection by enterococci was higher than that posed by E. coli. In Babolsar, the average annual infection risk caused by E. coli and enterococci was 0.365 and 1 for children and 0.181 and 0.986 for adults. Also, in Sari, the average annual infection risk caused by E. coli and enterococci was 0.060 and 0.908 for children and 0.027 and 0.815 for adults. In addition, children were more likely than adults to become infected. ConclusionIn light of the study's findings, due to the entry of untreated urban wastewater into the southern part of the Caspian Sea (northern Iran) and the high risk of infectious diseases for children, more control and health measures are necessary for children's swimming.

Site-specific risk-based threshold (RBT) concentrations for sewage-associated markers in estuarine swimming waters

This study establishes site-specific risk-based threshold (RBT) concentrations for sewage-associated markers, including Bacteroides HF183 (HF183), Lachnospiraceae Lachno3 (Lachno3), cross-assembly phage (CrAssphage), and pepper mild mottle virus (PMMoV), utilizing quantitative microbial risk assessment (QMRA) for recreational estuarine waters (EW). The QMRA model calculates a RBT concentration corresponding to a selected target illness risk for ingestion of EW contaminated with untreated sewage. RBT concentrations were estimated considering site-specific decay rates and concentrations of markers and reference pathogen (human norovirus; HNoV), aiding in the identification of high-risk days during the swimming season. Results indicated varying RBT concentrations for fresh (Day 0) and aged (Days 1 to 10) sewage contamination scenarios over 10 days. HF183 exhibited the highest RBT concentration (26,600 gene copis (GC)/100 mL) initially but decreased rapidly with aging (2570 to 3120 GC/100 mL on Day 10) depending on the decay rates, while Lachno3 and CrAssphage remained relatively stable. PMMoV, despite lower initial RBT (3920 GC/100 mL), exhibited increased RBT (4700 to 6440 GC/100 mL) with aging due to its slower decay rate compared to HNoV. Sensitivity analysis revealed HNoV concentrations as the most influential parameter. Comparison of marker concentrations in estuarine locations with RBT concentrations showed instances of marker exceedance, suggesting days of potential higher risks. The observed discrepancies between bacterial and viral marker concentrations in EW highlight the need for optimized sample concentration method and simultaneous measurement of multiple markers for enhanced risk predictions. Future research will explore the utility of multiple markers in risk management. Overall, this study contributes to better understanding human health risks in recreational waters, aiding regulators, and water quality managers in effective decision-making for risk prioritization and mitigation strategies.

Estimation and evaluation of the risks of protozoa infections associated to the water from a treatment plant in southern Brazil using the Quantitative Microbiological Risk Assessment Methodology (QMRA).

In this study, the Quantitative Microbial Risk Assessment (QMRA) methodology was applied to estimate the annual risk of Giardia and Cryptosporidium infection associated with a water treatment plant in southern Brazil. The efficiency of the treatment plant in removing protozoa and the effectiveness of the Brazilian legislation on microbiological protection were evaluated, emphasizing the relevance of implementing the QMRA in this context. Two distinct approaches were employed to estimate the mechanical removal of protozoa:The definitions provided by the United States Environmental Protection Agency (USEPA), and the model proposed by Neminski and Ongerth.Although the raw water collected had a higher concentration of Giardia cysts than Cryptosporidium oocysts, the estimated values for the annual risk of infection were significantly higher for Cryptosporidium than for Giardia. From a general perspective, the risk values of protozoa infection were either below or very near the limit set by the World Health Organization (WHO). In contrast, all the risk values of Cryptosporidium infection exceeded the threshold established by theUSEPA. Ultimately, it was concluded that the implementation of the QMRA methodology should be considered by the Brazilian authorities, as the requirements and guidelines provided by the Brazilian legislation proved to be insufficient to guarantee the microbiological safety of drinking water. In this context, the QMRA application can effectively contribute to the prevention and investigation of outbreaks of waterborne disease.

Quantitative Microbial Risk Assessment Framework Incorporating Water Ages with Legionella pneumophila Growth Rates.

Water age in drinking water systems is often used as a proxy for water quality but is rarely used as a direct input in assessing microbial risk. This study directly linked water ages in a premise plumbing system to concentrations of Legionella pneumophila via a growth model. In turn, the L. pneumophila concentrations were used for a quantitative microbial risk assessment to calculate the associated probabilities of infection (Pinf) and clinically severe illness (Pcsi) due to showering. Risk reductions achieved by purging devices, which reduce water age, were also quantified. The median annual Pinf exceeded the commonly used 1 in 10,000 (10-4) risk benchmark in all scenarios, but the median annual Pcsi was always 1-3 orders of magnitude below 10-4. The median annual Pcsi was lower in homes with two occupants (4.7 × 10-7) than with one occupant (7.5 × 10-7) due to more frequent use of water fixtures, which reduced water ages. The median annual Pcsi for homes with one occupant was reduced by 39-43% with scheduled purging 1-2 times per day. Smart purging devices, which purge only after a certain period of nonuse, maintained these lower annual Pcsi values while reducing additional water consumption by 45-62%.

Lessons identified from applications of the Risk Analysis Quality Test Release 1.0.

The Risk Analysis Quality Test Release 1.0 (RAQT1.0) was developed as a framework to encourage mutual understanding between technical risk analysts and risk management decision makers of risk assessment quality indicators. The initial version (release 1.0) was published by the Society for Risk Analysis (SRA) in 2020 with the intent of learning from early test applications whether the approach was useful and whether changes in approach or contents would be helpful. The results of applications across three diverse fields are reported here. The applications include both retrospective evaluations of past risk assessments and prospective guidance on the design of future risk assessment projects or systems. The fields represented include Quantitative Microbial Risk Assessment, Cultural Property Risk Analysis, and Software Development Cyber Risk Analysis. The RAQT1.0 proved helpful for identifying shortcomings in all applications. Ways in which the RAQT1.0 might be improved are also identified.

Scenario-based assessment of fecal pathogen sources affecting bathing water quality: novel treatment options to reduce norovirus and Campylobacter infection risks.

Wastewater discharge and runoff waters are significant sources of human and animal fecal microbes in surface waters. Human-derived fecal contamination of water is generally estimated to pose a greater risk to human health than animal fecal contamination, but animals may serve as reservoirs of zoonotic pathogens. In this study, quantitative microbial risk assessment (QMRA) tools were used to evaluate the hygienic impact of sewage effluents and runoff water from municipalities and animal farms on surface and bathing waters. The human-specific microbial source tracking (MST) marker HF183 was used to evaluate the dilution of fecal pathogens originating from the sewage effluent discharge to the downstream watershed. As novel risk management options, the efficiency of UV-LED disinfection and wetland treatment as well as biochar filtration was tested on-site for the contamination sources. According to the dilution pattern of the MST marker HF183, microbes from wastewater were diluted (2.3-3.7 log10) in the receiving waters. The scenario-based QMRA revealed, that the health risks posed by exposure to human-specific norovirus GII and zoonotic Campylobacter jejuni during the bathing events were evaluated. The risk for gastroenteritis was found to be elevated during wastewater contamination events, where especially norovirus GII infection risk increased (1-15 cases per day among 50 bathers) compared with the business as usual (BAU) situation (1 case per day). The noted C. jejuni infection risk was associated with animal farm contamination (1 case per day, versus 0.2-0.6 cases during BAU). Tertiary treatment of wastewater with wetland treatment and UV-LED disinfection effectively reduced the waterborne gastroenteritis risks associated with bathing. Based on the experiences from this study, a QMRA-based approach for health risk evaluations at bathing sites can be useful and is recommended for bathing site risk assessments in the future. In case of low pathogen numbers at the exposure sites, the MST marker HF183 could be used as a pathogen dilution coefficient for the watershed under evaluation. The full-scale implementation of novel tertiary treatment options at wastewater treatment plants (WWTPs) as well as on-site runoff water treatment options should be considered for infection risk management at locations where scenario-based QMRA implies elevated infection risks.

Using Quantitative Microbial Risk Assessment (QMRA) of SARS-CoV-2 to understand possible exposure to health risks in selected wastewater treatment plants located in the Eastern region of South Africa

In the past two years, Covid-19 has emerged as the most severe and pressing public health issue, causing a great deal of damage to societal and economic welfare, as well as causing illness and mortality. The operators in wastewater treatment plants (WWTPs), particularly those employed in rural communities, appear to often exhibit a lack of adherence to proper safety protocols by not utilizing sufficient protective equipment while handling unprocessed sewage samples throughout the different phases of wastewater treatment and disposal. This study aimed at examining the potential health risk of infection among WWTP operators, as a result of unintentional ingestion of wastewater during routine duties in facilities that receive influent containing Severe acute respiratory syndrome coronavirus 2 (SARS-COV-2) from various areas. This study examined the prevalence of SARS-CoV-2 in grab samples of untreated wastewater samples using the real-time quantitative polymerase chain reaction (RT-qPCR) technique and quantitative microbial risk assessment (QMRA) model was employed on three probable exposure of SARS-CoV-2 scenario that are expressed as moderate, aggressive and extreme (2 mL, 10 mL, 20 mL) to evaluate the probability of infection to WWTP workers based on the 6 hours that the workers spent in WWTP performing their daily activities which exposed them to potential health risk of various pathogens. At the highest SARS-CoV-2 genome of 266.23 × 102 gc/mL, the findings indicated that there was no statistically significant difference in the probability of infections with respect to seasonal differences because the P(i) value was greater than 0.05 (p>0.05). Overall, P(i) was highly significant across all volumetric scenarios in the study with p value that was p<0.001. The probability of getting infected during the different seasons is assumed to be low since there was no statically difference in P(i) with respect to season however it can be assumed that the is a high chance of getting infected regardless of volumetric intake. Our study suggests that the risk of accidental occupational exposure to SARS-CoV-2 in raw wastewater is negligible to workers whereby workers would perform their daily activities without wearing protective gear. Nevertheless, the importance and work of WWTP by workers should not be overlooked. Regardless of the situation, it is widely recognized that residential wastewater poses a potential risk of infection due to the presence of several enteric pathogens, therefore, it is crucial to ensure that those who are occupationally exposed to untreated wastewater are well equipped with suitable personal protective equipment (PPE).

One Health-based management for sustainably mitigating tetracycline-resistant Aeromonas hydrophila-induced health risk

Aeromonas hydrophila has ability to spread tetracycline resistance (tetR) under stresses of oxytetracycline (OTC), one of the most important antibiotics in aquaculture industry. Even though environmental reservoir of Aeromonas allows it to be at interfaces across One Health components, a robust modelling framework for rigorously assessing health risks is currently lacking. We proposed a One Health-based approach and leveraged recent advances in quantitative microbial risk assessment appraised by available dataset to interpret interactions at the human–animal–environment interfaces in various exposure scenarios. The dose–response models were constructed considering the effects on mortality for aquaculture species and tetR genes transfer for humans. A scenario-specific risk assessment on pond species-associated A. hydrophila infection and human gut-associated tetR genes transfer was examined. Risk-based control strategies were involved to test their effectiveness. We showed that farmed shrimp exposed to tetracycline-resistant A. hydrophila in OTC-contaminated water experienced higher infection risk (relative risk: 1.25–1.34). The tetR genes transfer risk for farmers in shrimp ponds (∼2×10−4) and swimmers in coastal areas (∼4×10−6) during autumn exceeded acceptable risk (10−6). This cautionary finding underscores the importance of accounting for monitoring, assessing, and mitigating occupational health hazards among workers in shrimp farming sectors within future One Health-based strategies for managing water infection risks. We recommend that OTC emission rate together with A. hydrophila concentration should be reduced by up to 70–99% to protect human, farmed shrimp, and environmental health. Our predictive framework can be adopted for other systems and be used as a “risk detector” for assessing tetR-related health risks that invoke potential risk management on addressing sustainable mitigation on offsetting residual OTC emission and tetR genes spread in a species–human–environmental health system.

Integrating quantitative microbiological risk assessment and disability-adjusted life years to evaluate the effects of urbanization on health risks for river recreationists

Urban rivers provide an excellent opportunity for water recreation. This study probabilistically assessed health risks associated with water recreation in urban rivers in the Bitan Scenic Area, Taiwan, by employing quantitative microbial risk assessment and disability-adjusted life years (DALYs). Moreover, the effects of urbanization on the health risks of river recreation induced by waterborne pathogenic Escherichia coli (E. coli) were investigated. First, data on river E. coli levels were collected in both the Bitan Scenic Area and the upstream river section, and model parameters were obtained through a questionnaire administered to river recreationists. Monte Carlo simulation was then employed to address parameter uncertainty. Finally, DALYs were calculated to quantify the cumulative effects in terms of potential life lost and years lived with disability. The results indicated that the 90 % confidence intervals for the disease burden (DB) were 0.2–74.1 × 10−6, 0.01–94.0 × 10−6, and 0.3–128.9 × 10−6 DALY per person per year (pppy) for canoeing, swimming, and fishing, respectively, in the Bitan Scenic Area. Furthermore, urbanization near the Bitan Scenic Area approximately doubled the DB risks to river recreationists in upstream rural areas. At the 95th percentile, the DB risks exceeded the tolerances recommended by the World Health Organization (1 × 10−6) or U.S. Environmental Protection Agency (1 × 10−4). The findings suggest that the simultaneous implementation of effluent sewer systems and best management practices can reduce health risks to river recreationists by at least half, reducing the DALY levels below 1 × 10−4 or even 1 × 10−5 pppy.

Assessing environmental exposure to viruses in wastewater treatment plant and swine farm scenarios with next-generation sequencing and occupational risk approaches

Occupational exposure to pathogens can pose health risks. This study investigates the viral exposure of workers in a wastewater treatment plant (WWTP) and a swine farm by analyzing aerosol and surfaces samples. Viral contamination was evaluated using quantitative polymerase chain reaction (qPCR) assays, and target enrichment sequencing (TES) was performed to identify the vertebrate viruses to which workers might be exposed. Additionally, Quantitative Microbial Risk Assessment (QMRA) was conducted to estimate the occupational risk associated with viral exposure for WWTP workers, choosing Human Adenovirus (HAdV) as the reference pathogen. In the swine farm, QMRA was performed as an extrapolation, considering a hypothetical zoonotic virus with characteristics similar to Porcine Adenovirus (PAdV). The modelled exposure routes included aerosol inhalation and oral ingestion through contaminated surfaces and hand-to-mouth contact.HAdV and PAdV were widespread viruses in the WWTP and the swine farm, respectively, by qPCR assays. TES identified human and other vertebrate viruses WWTP samples, including viruses from families such as Adenoviridae, Circoviridae, Orthoherpesviridae, Papillomaviridae, and Parvoviridae. In the swine farm, most of the identified vertebrate viruses were porcine viruses belonging to Adenoviridae, Astroviridae, Circoviridae, Herpesviridae, Papillomaviridae, Parvoviridae, Picornaviridae, and Retroviridae.QMRA analysis revealed noteworthy risks of viral infections for WWTP workers if safety measures are not taken. The probability of illness due to HAdV inhalation was higher in summer compared to winter, while the greatest risk from oral ingestion was observed in workspaces during winter. Swine farm QMRA simulation suggested a potential occupational risk in the case of exposure to a hypothetical zoonotic virus.This study provides valuable insights into WWTP and swine farm worker's occupational exposure to human and other vertebrate viruses. QMRA and NGS analyses conducted in this study will assist managers in making evidence-based decisions, facilitating the implementation of protection measures, and risk mitigation practices for workers.

The microbial and chemical risk analysis of drinking water in a small island, Spermonde Archipelago

Coastal areas face greater risk in terms of health and the environment. They are the most vulnerable to impacts resulting from climate change. Coastal areas with higher population density also have more environmental problems, such as natural disasters. Environmental health risks from chemicals and microbes continue threatening people living on small islands. Therefore, this study aims to: 1) conduct a chemical risk analysis of heavy metals Pb, Cr(VI), and Ni; 2) analyse the microbial risk posed by drinking water consumed daily by people on small islands. A method used to analyse the chemical risk of heavy metals was the environment health risk assessment (EHRA), whereas to analyse the microbial risk in small islands, the quantitative microbial risk assessment (QMRA) was used. The results showed that the concentration of heavy metals in drinking water was &amp;lt;0.0012 mg∙dm–3 for Pb, &amp;lt;0.01 mg∙dm–3 for Cr(VI), and &amp;lt;0.0019 mg∙dm–3 for Ni. The three heavy metals showed worrying results. Assessment and obtained risk quotient were less than one (RQ &amp;lt; 1) in all samples. Meanwhile, the microbial analysis found Escherichia coli, Acinetobacter calcoaceticus, Enterobacter sp ., and Citrobacter sp ., with risk characterised from low to high. Risk management is needed to control environmental health risks posed by heavy metals and the microbiological characteristics of drinking water on the small islands of the Spermonde Archipelago.

Public health risk assessment and speciation of air-borne microorganisms in an office building

This study focuses on speciation of air borne microbes in an office building and health impacts of these microbes in terms of Quantitative Microbial Risk Assessment (QMRA), Hazard Quotient (HQ) and Hazard Index (HI). This is important in the wake of Sustainability Development Goal 3 (Public Health and well-being) specially when there is a lack of standards in terms of microbial air quality in office buildings where people spend 8–10 h indoors. Air borne bacterial and fungal species in different floors of the office building with varying occupancy and work profile were identified. The load of bacterial count on an average for the entire building was found to be 1917 CFU/m3. Overall, Gram-positive bacteria were predominant (>53%). Based on V3–V4 sequencing Staphylococcus scuiri (causing UTIs) was the most abundant (93.7%) and other bacterial species found were Lactobacillus hamsteri, Prevotella copri, Bacteroides plebeius Faecalibacterium prausnitzii, Bacillus coagulans (gut commensals), Shigella boydii (causes bacillary dysentery), and Propionibacterium acnes (acne producing). Fungal load was 4000 CFU/m3. Based on ITS sequencing Aspergillus (45.6%) was the dominant fungus and other fungi found were Cunninghamella, Lichtheimia, Fusarium, and Circinella Grammothele, Chondrostereum, and Pseudolagarobasidium, Candida sp., Chondrostereum, Sarocladium. The QMRA of gram-negative air borne bacteria showed a high disease burden, well over the WHO benchmark values even though Hazard Quotient (HQ) and Hazard Index (HI) was found to be negligible. These findings can contribute to the development of guidelines for seemingly safe workplaces that may harbour disease-causing microbes and calls for an immediate attention from policy makers as it is a major cause of concern for public health.

Eye infection risks from Pseudomonas aeruginosa via hand soap and eye drops.

Eye infections from bacterial contamination of bulk-refillable liquid soap dispensers and artificial tear eye drops continue to occur, resulting in adverse health outcomes that include impaired vision or eye enucleation. Pseudomonas aeruginosa (P. aeruginosa), a common cause of eye infections, can grow in eye drop containers and refillable soap dispensers to high numbers. To assess the risk of eye infection, a quantitative microbial risk assessment for P. aeruginosa was conducted to predict the probability of an eye infection for two potential exposure scenarios: (i) individuals using bacteria-contaminated eye drops and (ii) contact lens wearers washing their hands with bacteria-contaminated liquid soap prior to placing the lens. The median risk of an eye infection using contaminated eye drops and hand soap for both single and multiple exposure events (per day) ranged from 10-1 to 10-4, with contaminated eye drops having the greater risk. The concentration of P. aeruginosa was identified as the parameter contributing the greatest variance on eye infection risk; therefore, the prevalence and level of bacterial contamination of the product would have the greatest influence on health risk. Using eye drops in a single-use container or with preservatives can mitigate bacterial growth, and using non-refillable soap dispensers is recommended to reduce contamination of hand soap. Given the opportunistic nature of P. aeruginosa and its ability to thrive in unique environments, additional safeguards to mitigate bacterial growth and exposure are warranted.IMPORTANCEPseudomonas aeruginosa (P. aeruginosa) is a pathogen that can persist in a variety of unusual environments and continues to pose a significant risk for public health. This quantitative microbial risk assessment (QMRA) estimates the potential human health risks, specifically for eye infections, associated with exposure to P. aeruginosa in bacteria-contaminated artificial tear eye drops and hand soap. This study applies the risk assessment framework of QMRA to evaluate eye infection risks through both consumer products. The study examines the prevalence of this pathogen in eye drops and soap, as well as the critical need to implement measures that will mitigate bacterial exposure (e.g., single-use soap dispensers and eye drops with preservatives). Additionally, limitations and challenges are discussed, including the need to incorporate data regarding consumer practices, which may improve exposure assessments and health risk estimates.

Detection and quantitative microbial risk assessment of pathogenic Vibrio cholerae in a river used for drinking, domestic, fresh produce irrigation and recreational purposes

Cholera infection results from the ingestion of water or food contaminated with toxigenic Vibrio cholerae. This study evaluated the occurrence of toxigenic V. cholerae in Asata River, Enugu Metropolis, Nigeria, and estimated V. cholerae infection risks from use of the River water for drinking, domestic and recreational purposes. Vibrio was detected and quantified using membrane filtration technique and thiosulfate–citrate–bile salts–sucrose agar. Isolates were screened by PCR, using specific primers targeting the internal transcribed spacer (its) region between 16 and 23S rRNA and the cholera toxin (ctx) gene. Sequenced 16SrRNA gene amplicons of two selected isolates were used for phylogenetic analysis. Quantitative microbial risk assessment (QMRA) was conducted using the β-Poisson dose–response model. About 81% (58/72) of Asata River samples recorded Vibrio counts above 1.0 ×\\documentclass[12pt]{minimal} \\usepackage{amsmath} \\usepackage{wasysym} \\usepackage{amsfonts} \\usepackage{amssymb} \\usepackage{amsbsy} \\usepackage{mathrsfs} \\usepackage{upgreek} \\setlength{\\oddsidemargin}{-69pt} \\begin{document}$$\ imes$$\\end{document} 103 cfu/100 mL. Of the fifty Vibrio isolates screened, its was detected in 54% (27/50), out of which 74% (20/27) had the ctx gene of toxigenic V. cholerae. Evolutionary relatedness of the sequenced isolates to V. cholerae was revealed. The estimated risks of cholera infection in persons exposed to untreated Asata River water were above 0.5 for all the exposure scenarios, for both the rainy and dry seasons. The risks were highest (~ 0.9) for exposure via drinking water and annual risk of infection was deduced to have a probability of 1.0. Therefore, dependence on the untreated Asata River water for drinking, recreational, domestic and irrigation purposes may present a potential public health risk of cholera outbreak. We recommend increased monitoring and surveillance of River water for Vibrio abundance and that Asata River be protected from further degradation.

Assessing sustainability of microalgae-based wastewater treatment: Environmental considerations and impacts on human health

An integrated life cycle assessment (LCA) and quantitative microbial risk assessment (QMRA) were conducted to assess microalgae-mediated wastewater disinfection (M-WWD). M-WWD was achieved by replacing ultraviolet disinfection with a microalgal open raceway pond in an existing sewage treatment plant (STP) in India. Regarding impacts on human health, both M-WWD and STP yielded comparable life cycle impacts, around 0.01 disability-adjusted life years (DALYs) per person per year. However, QMRA impacts for M-WWD (0.053 DALYs per person per year) were slightly lower than that for STP while considering exposure to E. coli O157:H7 and adenovirus. Additionally, a comparative LCA resolved the dilemma about the appropriate utilization of microalgal biomass. Among biodiesel, biocrude, and biogas production, the lowest impacts of 0.015 DALYs per person per year were obtained for biocrude for 1 m3 water treated by M-WWD. Electricity consumption in microalgae cultivation was a major environmental hotspot. Overall, M-WWD, followed by production of microalgal biocrude, emerged as a sustainable alternative from environmental and public health perspectives. These findings set the foundation for pilot-scale M-WWD system development, testing, and economic evaluation. Such comprehensive investigations, encompassing LCA, QMRA, and resource recovery scenarios, offer crucial insights for stakeholders and decision-makers in wastewater treatment and environmental management.

Quantitative assessment of food safety interventions for Campylobacter spp. and Salmonella spp. along the chicken meat supply chain in Burkina Faso and Ethiopia

Rural and small-scale chicken farming is a major source of income in most African countries, and chicken meat is an important source of nutrients. However, chicken meat can be contaminated with Campylobacter spp. and Salmonella spp., pathogens with a high reported burden of foodborne illnesses. Therefore, it is essential to control these pathogens in chicken meat. Quantitative microbial risk assessments (QMRA) can aid the development of effective food safety control measures and are currently lacking in chicken meat supply chains in the African context. In this study, we developed stochastic QMRA models for Salmonella spp. and Campylobacter spp. in the chicken meat supply chain in Burkina Faso and Ethiopia employing the modular process risk model in @Risk software. The study scope covered chicken farming, transport, slaughtering, consumer handling, and consumption. Effectiveness of candidate interventions was assessed against baseline models' outputs, which showed that the mean annual Campylobacter spp. risk estimates were 6482 cases of illness per 100,000 persons and 164 disability adjusted life years (DALYs) per 100,000 persons in Burkina Faso, and 12,145 cases and 272 DALYs per 100,000 persons in Ethiopia. For Salmonella spp., mean annual estimates were 2713 cases and 1212 DALYs per 100,000 persons in Burkina Faso, and 4745 cases and 432 DALYs per 100,000 persons in Ethiopia. Combining interventions (improved hand washing plus designated kitchen utensils plus improved cooking) resulted in 75 % risk reduction in Burkina Faso at restaurants and 93 to 94 % in Ethiopia at homes for both Salmonella spp. and Campylobacter spp. For Burkina Faso, adding good hygienic slaughter practices at the market to these combined interventions led to over 91 % microbial risk reduction. Interventions that involved multiple food safety actions in a particular step of the supply chain or combining different interventions from different steps of the supply chain resulted in more risk reduction than individual action interventions. Overall, this study demonstrates how diverse and scanty food supply chain information can be applied in QMRA to provide estimates that can be used to stimulate risk-based food safety action in African countries.

Human health risk assessment framework for new water resource recovery-based bio-composite materials.

A new type of bio-composite material is being produced from water-recovered resources such as cellulose fibres from wastewater, calcite from the drinking water softening process, and grass and reed from waterboard sites. These raw materials may be contaminated with pathogens and chemicals such as Escherichia coli, heavy metals, and resin compounds. A novel risk assessment framework is proposed here, addressing human health risks during the production of new bio-composite materials. The developed framework consists of a combination of existing risk assessment methods and is based on three main steps: hazard identification, qualitative risk mapping, and quantitative risk assessment. The HAZOP and Event Tree Analysis methodologies were used for hazard identification and risk mapping stages. Then, human health risks were quantitatively assessed using quantitative chemical risk assessment, evaluating cancer and non-cancer risk, and quantitative microbial risk assessment. The deterministic and the stochastic approaches were performed for this purpose. The contamination of raw materials may pose human health concerns, resulting in cancer risk above the threshold. Microbial risk is also above the safety threshold. Additional analysis would be significant as future research to better assess the microbial risk in biocomposite production. The framework has been effectively used for chemical and microbial risk assessment.
